Setting up a form for your web page is very simple.
You can use forms so visitors to your site will have a quick, easy way to contact you without them having to use an e-mail client. Our PolyForm program can e-mail you when someone fills out your form.
The only steps needed are for you to insert a form into your HTML document, and for the form action you will be using the syntax below, and it will be POSTed.
You can have as many fields in your form as you would like. We recommend you use Name, E-Mail, Subject, and Comments fields for your visitors to fill out if they have any questions.
The Syntax
<form action="http://www.your-domain.com/cgi-win/polyform.exe/form-name" method="POST">
Where:
- your-domain represents your domain name (eg. www.yourcompany.com)
- form-name is a name that we will give to your page's form, usually we will use your company's name.
All you should have to do is make a form on your HTML document, making sure you use the above example. Once you have made a form, call us and we will set up a form for you in a matter of minutes. Use the form-name that we gave you in your <FORM> HTML tag. You can use any standard HTML features such as radio buttons, text entry boxes, selection bars, and all sorts of things like that. Forms are not interactive, so please don't think you'll be able to put some sort of shopping mall on your web site with our PolyForm program.
